Free Michigan Power of Attorney Forms

Financial Power of Attorney (Durable)
A Financial Power of Attorney lets you appoint someone you trust to handle your finances if you can’t do it yourself. This can include paying bills, managing bank accounts, selling property, and other money-related matters.
Key features:
- Durable: stays in effect even if you become incapacitated.
- Customizable: you decide what powers your agent has.
- Revocable: you can cancel it at any time while you still have capacity.
Medical Power of Attorney (Patient Advocate Designation)
A Medical Power of Attorney allows you to choose a trusted person to make healthcare decisions on your behalf if you can’t communicate with them yourself. In Michigan, this is formally called a Patient Advocate Designation.
Key features:
- Healthcare choices: covers treatment decisions, living arrangements, and end-of-life care.
- Personal values: ensures your medical care follows your wishes.
- Activation: only takes effect if you are unable to make your own medical decisions.
How to Use These Forms in Michigan:
Fill out the form completely, naming your chosen agent(s).
Sign in blue or black ink.
For financial POA - must be notarized in Michigan.
For medical POA - must be signed in front of two witnesses (cannot be your agent, a healthcare provider, or an employee of the facility where you receive care).
Provide copies to your agents and keep the originals in a safe place.
